MARSHFIELD, WI (OnFocus) – With no end in sight of the COVID-19 pandemic, more and more places have been requiring proof of a COVID-19 vaccination or other vaccinations. This means it is very important to keep your vaccination records straight.

What if I lost my vaccination card?

After you get your first dose (or only dose) of either the mRNA or Johnson & Johnson COVID-19 vaccine, you are given a vaccination card that details which vaccines have been administered to you. These cards are what some places and employers are asking to see.

If you have lost this card, you have some options to obtain proof of vaccination against the COVID-19 virus.

You can get a copy of your vaccination records by logging onto the Wisconsin Immunization Registry. This is the quickest and most efficient way of getting your proof of vaccination.

You can also print directly from this page and it has an option that allows you to see what immunizations are recommended to get right now and which ones you are current on.

Another way is to call the organization or provider where you got the vaccine from. They might be able to provide a copy of your vaccination record.

Once you have a replacement card, taking a picture of the card and having it on your phone is the easiest way to ensure the card is with you.

It is recommended by the Wisconsin Department of Health Services that you do not post pictures of your vaccination card online and that you do not laminate the card.
